USS Scorpion was one, it's possible it was following a Russian sub which then performed an evasive manoeuvre resulting in collision. Oh yeh and there is up to 50 nuclear weapons that are lost not to mention the nuclear reactors from those subs that cannot be retrieved In the 60s a B52 bomber had to drop 4 nukes in an emergency, 2 were recovered, 1 exploded showering the area with plutonium (apparently 5 out of 6 safety fuses failed which mean it almost fully detonated on US soil) and the 4th landed in a swamp and it's still there today crazy!
